A change that did not apply shows as Failed

When the agent tries to change a file and the change does not go through, the chat now marks that step Failed in red.

• Before: a failed edit looked like a finished one, with line counts and a preview of the change, while the file stayed the same.
• Now: the step shows Failed and no line counts. Open it and it says the step did not go through. Steps that worked look as before.
