Choose: Use the workflow the clinic already operates reliably. Start with a pending request when staff decides availability; use a provider when it already owns the schedule and its account, plan, credentials, data terms, and failure behavior are verified.
Tradeoff: A custom request minimizes fields and keeps the boundary clear but requires staff follow-up. A provider may reserve real slots while adding separate privacy terms, configuration, credentials, interface constraints, costs, and outage modes.
